No-cost health insurance for adults with disabilities or age 65+


STAR+PLUS is a Texas Medicaid program for adults with disabilities or individuals age 65 or older. It covers your medical needs, like doctor visits and hospital care, and offers services and support to help you live where you want to live.

What you get with Wellpoint


We provide services to help you live and get care in a place you call home. This includes:

  • Help with daily activities such as making meals or cleaning.
  • Help to live safely in your home, like installing grab bars in the shower or providing emergency response systems.
  • Employment assistance and supported employment.
  • Assisted living and adult day services.

You have options to manage your service attendants:

  • If you want to do it yourself, you can be in control.
  • If you want to have someone manage it for you, you can use an agency.
  • If you use one of our agencies, we have electronic verification to make sure your caregivers are coming and giving you the care you need.

You can have your own service coordinator who will:

  • Learn about your health-care needs and goals.
  • Work with you to help create a care plan that works for you.
  • Help you make appointments.
  • Coordinate with all of your providers.
  • Answer any questions you have.

You will always have a familiar name you can reach out to when you need something

If you live in a nursing facility and want to stay, we can help you there. Each nursing facility we work with has one of our own care coordinators assigned. They can also help if you are ready to move back home.

If you live in a nursing facility and want to move back to a home, a service coordinator can help. Through the Money Follows the Person program, we may be able to help with:

  • Security deposits or rent payments
  • Startup expenses for utilities like water, gas or electricity
  • Furniture
  • Home modifications

If you do not have Medicare, we cover your primary medical needs, too. This includes doctor, hospital and urgent care visits, prescriptions, specialist visits, therapy and much more.

If you have Medicare, your Medicare plan will cover most of these services.

Health issues come up when you least expect them. You can get fast, free medical advice from a registered nurse anytime, day or night.

  • Rewards for doing healthy activities like getting certain checkups or treatments
  • Cellphone or smartphone with monthly minutes, data, and texts
  • Pest control services once every 3 months
  • Extra respite care hours for family members and caregivers
  • Up to 20 home-delivered meals after leaving a hospital or nursing facility
  • GED testing fee reimbursement

Do you qualify?


You may qualify if you are eligible for Medicaid and meet at least one of the following:

Age 21 or older and get Supplemental Security Income (SSI) benefits

Don’t get SSI, but are able to get STAR+PLUS Home and Community-Based Waiver Services

Age 21 or older and get Medicaid through Social Security Exclusion programs 

Age 21 or older and live in a nursing home

Get services through the Medicaid Breast and Cervical Cancer (MBCC) program

Enrollment in STAR+PLUS is year round. You can apply at any time.

Print or request a paper form and submit by mail

Names, birth dates and Social Security numbers (or document numbers for any legal immigrants who need insurance)

Job and income information, including: Unemployment benefits, Social Security benefits, pensions, retirement income, rental income, alimony received, etc.

Citizenship or immigration status

Tax filing information

Deductions like alimony paid, self-employment expenses, school expenses, etc.

Current health insurance information, if any

Choose Wellpoint as your health plan

After you are approved, you will get an enrollment packet in the mail from the Texas Health and Human Services Commission (HHSC).
