What you get with Wellpoint

Get the care you need from a doctor you choose. We have thousands of doctors in our plan, including many specialists.
When you have to get care in the hospital, you’re covered. Benefits also include urgent and emergency care.
You get regular checkups, called wellness visits, to help you stay healthy. This also includes routine shots and screenings.

Healthy eyes and teeth are an important part of good overall health.

 

Members under 21 can get:

  • Eye exam and glasses once a year
  • Dental checkups, cleanings, and treatments

 

Adults can get:

  • Medically necessary vision care
  • Emergency dental services
Your prescription benefits are covered through the state.

Gather up this important information for you and any family members who are applying for health care coverage:

Names, birth dates, and Social Security numbers

Most recent federal tax information (if available)

Pay stubs or other job and income information for the last month

Other income like unemployment benefits, SSI benefits, pensions or retirement, alimony, etc.

Current health insurance information, if any

Apply for benefits in one of these ways:

Go to wvpath.wv.gov and create an account to start your application

Call the DHHR Customer Service Center at 877-716-1212

Visit your local DHHR Office

Once you’re approved, you’ll be assigned to a health plan and get a notice in the mail. If you were assigned to Wellpoint, you don’t have to do anything to stay with us.

You can change your health plan at any time in one of these ways:

Call Mountain Health Trust at 800-449-8466 (TTY 304-344-0015), Monday through Friday from 8:30 a.m. to 4:30 p.m.

Fill out and return the forms you received in the packet you received in the mail with your notice
