- Up to $2,500 reintegration benefit to assist eligible members moving from a facility to home-based setting

How to join Wellpoint
Apply for benefits
Medicaid is open to adults and families who meet certain income requirements, pregnant women and new moms, and people with disabilities.
- You can apply for benefits at any time. Enrollment is year-roud.
- Visit the Iowa Department of Health and Human Services (HHS) website to check your eligibility and apply for benefits online.
After you are approved for benefits, you’ll be assigned to a health plan. You can choose a different health plan using one of the ways listed below.

Email your completed managed care organization (MCO) change form to Iowa Medicaid Member Services.
